Across British Columbia, family practice divisions, health authorities, First Nations, and community partners are collaborating to create Primary Care Networks (PCNs).

A Primary Care Network (PCN) consists of local family physicians and other primary care and social service providers within a community. PCNs emphasize a team-based approach to primary care, built on the foundation of Patient Medical Homes (PMHs).

This patient-centered model simplifies collaboration among family physicians and improves access to resources and support, ensuring patients receive the care they need. For patients, PCNs enhance access to primary care and other health services.

An integrated system brings together GPs and patients. This is better for Physicians as the focus shifts towards diagnoses and patient relationships and it is better for patients as it increases long term attachment to a primary care provider. The Langley Patient Care Network provides a broad range of services such as Clinical Counsellors, Support in Mental Health and Substance Abuse, Elderly Services, Indigenous Services, Physiotherapy, and more.

A Primary Care Network (PCN) in British Columbia enhances patient care by providing a comprehensive, team-based approach to health services. As a patient, you gain access to a wide range of integrated support, including mental health services, where clinical counsellors and psychologists can help manage stress, anxiety, and depression. For older adults, elderly care services ensure aging-related health concerns are addressed proactively. The Langley PCN brings together the community of Langley, its Primary Care Providers, and Fraser Health to ensure patients have access to quality primary health care that effectively meets their needs.
