
The Langley PCN is a Ministry of Health funded opportunity for our Community Primary Care Providers, local Health Authority and community partners to work collaboratively on creating access to care, reducing wait times and building a team-based care approach to your wellbeing.
PCNs are different from community to community. In Langley, our work has been focused on building resources, education and capacity for community members who may not be able to access services otherwise. Following community feedback, we have focused our resources on serving gaps in care for mental health, frail and elderly, Indigenous care and navigational tools.
