Fragility Fracture Prevention Program

The PCN Fragility Fracture Prevention Program offer Langley community members one-on-one education support with a Registered Nurse & Pharmacist. Offering one to four in-person or virtual appointments, Your pharmacist will review your medication to determine if any put you at risk for falls. Your registered nurse will provide an opportunity to ask about your bone health history, previous fractures and work with you and your family care provider to prevent future fractures and keep your bones strong.
Fragility Fracture: a previous fracture in adult life occurring spontaneously, or a fracture arising from low impact trauma which, in a healthy individual, would not have resulted in a fracture. Example: falling from a standing height, and fracturing the forearm.
Eligibility
Please contact your Langley Family Physician or Nurse Practitioner for more information and referrals for the Fragility Fracture Prevention Program if you:
Diagnosed with Osteoporosis
Taking Osteoporosis medication
General at-risk population
Perimenopausal and menopausal age 50+
Frailty
Parental hx of hip fractures
Androgen deficiency
Rheumatoid arthritis
History of a fragility fracture or multiple
