Social Work Services

Our PCN Registered Social Worker offers Langley community members one-on-one navigational and system support. While offering three to six in-person or virtual appointments, our Social Worker is available to assist with government support forms (housing, disability, financial…), isolation concerns, and connecting you with appropriate community care. 

Eligibility

• Patient is attached to a Langley Family Physician or Nurse Practitioner

• Patient is 18 years or older

• Patient is suspected to fall within the mild to moderate range

• Patient cannot access service through other measures (e.g. WCB, ICBC, extended health benefits)
