Fragility Fracture Prevention Program
The PCN Fragility Fracture Prevention Program offers Langley community members one-on-one education support with a Registered Nurse & Pharmacist. Offering one to four in-person or virtual appointments, Your pharmacist will review your medication to determine if any put you at risk for falls. Your registered nurse will provide an opportunity to ask about your bone health history, previous fractures and work with you and your family care provider to prevent future fractures and keep your bones strong.
Fragility Fracture: a previous fracture in adult life occurring spontaneously, or a fracture arising from low impact trauma which, in a healthy individual, would not have resulted in a fracture. Example: falling from a standing height, and fracturing the forearm.
