Metabolic Health and Lifestyle Program

The PCN Metabolic Health and Lifestyle Program provides personalized, one-on-one support for people with health concerns such as high blood pressure, high blood sugar or unhealthy cholesterol levels. You will work with a nurse, pharmacist, dietitian and clinical exercise physiologist to make lifestyle changes and manage your health.

The team will review your health, offer guidance on healthy eating, physical activity and medications, and provide follow-up support. Follow-up appointments take place at 6 weeks, 3 months, 6 months and 12 months to support your progress. The goal is to help you make lifestyle changes that can prevent or better manage conditions such as diabetes and heart disease.
